View properties of the application pool in IIS manager. Navigate to the "Performance" tab.

Under Web garden, make sure Maximum number of worker processes is ALWAYS "1" . Changing this to a value above 1 is not allowed with Sitecore installations as it will result in multiple inconsistent Sitecore caches.

NOTE: On multiple processor servers, this value may automatically be set higher than one during the process of OS/IIS installation. On these servers it is therefore especially important to check this setting as it will likely be higher than "1".
